on a rig a couple miles from us the portable BOP accumulator exploded.it was cold this morning -25 f. they started the accumulator checked ram function while it wasn't on the well. went to the dog house to warm up. apparently the pressure kill didn't shut it down.at 1500psi.while they were in the doghouse the accumulator exploded launching two bottles off throwing shrappnel in all directions . no one was hurt. it also has a popoff which didn't work.  the manifold  holding the bottles is built for 7000 psi. <br>

generally a relief valve is in place to prevent failures like this.. Most accumulators also have an electric pressure switch that should kill the electric pump at a set pressure.. air pumps are the same way.. they have an air pressure switch.. set to kill air pumps @ 2700 psi and elect pumps kill @ 3000.. If it doesnt have dual pump types like described it has on or the other.. and a hydroseal relief valve come in 3200 or 3500 psi.. during rig moves if the bottles got whack it could crack the nipple that connect bottles to bottle manifold.. even if they are high pressure nips. Even then the U shaped bottle brackets should have kept the bottles out of orbit even though ive seen cases where this is not reality either... So this accident went down as follows:

Hand turns electric operating switch to the "on" position instead of Auto, or the cold weather effects the cut out setting. ( this happens alot.. but not to this exent)...or if air pumps.. water content of hydraulic fluid froze and didnt allow pressure reading to air pressure switch/or electric pressure switch thru the 1/4 stainless inlet and it has no input to shut off air to air pumps or electric pumps...or on most accumulators there is an air pump bypass for testing BOP.. a hold over of an old design ...NABORS AND PATTERSON HAVE THEM REMOVED !! Hand goes to doghouse.. pressure relief valve removed or inoperable.. coroded or damaged bottle nipples let loose.. no bottle brackets or defective bottle brackets.. houston.. we have a problem...


This illustration of this accident shows it happened either by defective or improper care of Koomey, lack of training.. ect.. if one of these elements described above were removed from the equation then the accident wouldnt have happened.. thats why little things are important too...

Could water in the airline have froze? Maybe no Methanol added, or water drained daily? Just a thought

frozen water in the airline would make pumps slower or inoperable .. but would not cause an incident like this .. ethylene glycol should be added to accumulators hydraulic fluid in 25% ration of total tank capacity..

these are portable trailer mounted. powered by a 2 cylinder onan . you start the  motor and a murphy switch kills it at about 1500psi. i've seen them go to 1800 psi in cold weather before they shut down. they have an air pump but running a 1/4" air line from the rig generally won't operate it in cold weather. i was supprised  one of the hoses didn't blow. they just have 1/2" hoses to the bops.or the belts running the pump would have slipped .they are rentals so how they service them is unknown . they come out nice and clean and painted. the company has been in business over 25 years and it never happened before.anyway there is a big root cause going on . i imagine someone will have to babysit them from now on so they are standing next to it when it blows.

what brand closing unit was this? Type '80, CAD, T3 ?
